Illumination Foundation offers targeted, interdisciplinary services for the most vulnerable clients in order to break the cycle of homelessness in Orange, Los Angeles, and San Bernardino County California.

Funds will support the expansion of a Recuperative Care Pilot Program, offering homeless hospital discharge services. Additional funds will support staff training on trauma-informed care, plus wellness and relaxation activities for direct care staff.

Manatee County Medical Society Foundation is the charitable arm of Manatee County Medical Society whose mission is to guard and foster the interests of Manatee County physicians and to promote high-quality health care for patients.

Funds will support the launch of a LifeBridge Physician Wellness program, offering physicians access to confidential counseling and life coaching services, wellness activities, and other support services.

San Francisco Community Clinic ConsortiumSan Francisco Community Clinic Consortium


San Francisco Community Clinic Consortium (SFCCC) develops innovative programs to ensure that people of all income levels have access to health care that is comprehensive, coordinated, and efficient; culturally and linguistically appropriate while focused on patient needs.

Funds will support Street Outreach Services (SOS), connecting clients with primary and specialty care, mental health, substance abuse, and other services.
